All applicants are required to use the electronic visa application form (DS-156). All applicants failing to present the electronic visa application form will be asked to schedule a new application appointment enabling them to return with the appropriate form. All information entered online is contained in a barcode - printed on the last page of the completed application form - which also needs to be presented at the time of the application.
