All applicants are required to use the electronic visa application form (DS-156). The form MUST be completed on the PC, and printed out afterwards. All applicants failing to present the electronic visa application form or present it with broken barcode will be asked to schedule a new application appointment enabling them to return with the appropriate form. All information entered online is contained in a barcode - printed on the last page of the completed application form - which also needs to be presented at the time of the application.
